Here are some things mothers have found helpful. 
1. Lots of skin to skin holding with baby, regardless of "feeding time", 
just let the baby get used to the feel of mom's bare skin.
2. Letting baby begin the feed with the shield, then removing it after the 
first couple of minutes. When baby pauses, mom can lean away from him and 
flick the shield off and then put him quickly back onto the breast.
3. Pumping or massaging to get letdown before putting baby on naked 
nipple. 
4. Counsel mom that this takes a lot of patience and that she shouldn't be 
discouraged if it takes a few weeks before they are nursing all 
feeds "naked". 
5. Celebrate with mom that she has a full supply and healthy gaining baby. 
Remind her that she is having success right now with nursing. 
6. Remind mom that if the baby gets frustrated at any time, it is ok to 
put the shield back on and try again later. 
7. Mom should never wait for baby to be "hungry enough" to just take what 
she offers. He may develop an aversion to the breast because it is 
presented when he is already grumpy. 

These are general tips, it will be helpful to know why mom started with 
the shield in the first place to get more specific with recommendations 
tailored to her unique situation. If you can't find out without making her 
feel like your questions are criticizing her decision, just start where 
you are.
