My guess would be ductal ectasia - here is a little info about signs and
symptoms from a google search about this:

   - A dirty white, greenish or black nipple discharge
   - Tenderness in the nipple or surrounding breast tissue
   - Redness
   - A lump or thickening
   - An inverted nipple

It could also be mastitis. However, since you mentioned "scab" there would
be a possibility of Paget's disease of the nipple, a form of breast cancer.
Mother should therefore see her doctor for an evaluation.
