I have seen several moms with a normal baseline and no surge who make very little milk. I do not think I ever tried Domperidone in that situation, as I would not expect it to help.



Unfortunately,  there is A LOT we do not understand about human lactation. I once saw a woman with a prolactin level in the 20s at 2 weeks who had plenty of milk.



So I just explain to mom it looks like her body is not responding to suckling like it is supposed to, it is nothing she did or did not do,  and I would focus on getting the baby to use a supplementer at the breast for the long term.
