If you can get cabbage, try using clean raw crisp cabbage leaves in the bra  
between feedings. If not, use cold packs  (bags of crushed ice, frozen  
vegetables or a towel, soaked and wrung out, placed in the freezer for about 10  
minutes bfore using) . Stop the heat/warm compresses except for right  before 
trying to pump- briefly no more than 5 minutes.. Also try a  basin of warm water 
and leaning over, soaking both breasts at the same time to  help milk leak 
immediately prior to pumping. I've also used breast shells to  encourage leaking 
to relieve some of the pressure. Have mom wear them with  nursing pads 
covering the lower half of the shell only.
 
Once you get a good milk flow, continue to use cabbage or cool compresses  to 
reduce swelling for at least 24 hours. This lady's milk supply may be  
seriously compromised if this is not resolved soon.The cabbage leaves should be  
changed when they get soggy.
