I asked for help from the group with examples of informed consents that
you use for rubber nipples and for formula supplementation and I got a
great response.  We have presented the consents to staff and are gearing
up to present them to physicians and we have met some resistance from
staff.  My question to those of you using consents for formula
supplementation, Do you use them for breastfeeding moms only or do you
also do a Formula consent for Formula feeding Only moms?  Some staff
believe that anyone choosing formula should get this consent and I would
just like to know what everyone else is doing?  Thanks so much in
advance!
