I just finished a 1 year study look up for a drug research class resulting in
a 10 page paper on the comparison and safety of using both of these
medications of mothers of premature infants -- using real study information
-- all i know so far that i got a 90% on this final term paper for my class
-- when i get the full low down & clean up the grammar a bit  --  i will be
able to make if available to interested persons.

-- some of my final thoughts that i pulled together -- with the limited
"good" info that was available (after slogging through a years worth of
research)-- domperidone is safer and just as effective in this preemie Mom
situation with low or dropping supply and that on a side note that -- the
compounded product does not seem to work as effectively as the OTC versions
that mothers can get themselves from non US sources.  That are many and
varied including rx from Canada (unless the US figures a way to stop citizens
from getting it themselves.

remember we can not prescribe -- "suggest" or diagnose anything

-- but there is no laws that i know of that prohibit moms from searching
reliable Internet resources (if a few URLs slip out -- so be it) to educate
themselves on what they might do after a full and complete one on one
evaluation with a board certified lactation consultant  -- checking for all
of the normal things that are in our scope of practice -- latch - baby
assessment -- full and complete and frequent milk removal -- either by a
"real"  breast pump or by hand expression  which is always the way to go
before going to drugs or herbs to help boost supply --

-- i must say that i am getting very scared by the number of doctors who will
prescribe reglan to boost supply -- without a Mom baby evaluation at all or
by really warning mom's of the possible side effects of the drug -- i have
seen many scary situations over the past 15 years -- this is why in "my"
practice reglan is a drug of last choice and only for a very few moms -- who
are the least likely to have the most common side effects.


-- My rule is move the milk 1st -- either by baby or expression -- look for
breast and mouth (lock and key) mismatches --  then moms overall living
situation (it can have a very "real" affect on what happens with supply)  i
could list 40-50 more situations to evaluate for before going to drugs.  but
we as practitioners need to look at the whole picture -- i am sure that the
many sages (I hesitate to say old pros) on this list would tend to agree.

--hey i have only been at this full force since 1987 and started my search
for knowledge in the late 1970s -- long before it was popular -- i was just a
young punk kid with ODD non nurse ideas -- (hey my Mom was a labor and
delivery nurse when i was born and she was an ob clinical educator when i was
in high school -- i just could not escape the baby bug -- so sue me.
