That specific part you pointed out sounds more like a in-hospital type 
situation.  Thats the only place I can see it applied.  Where you would make 
notes in the chart.  Other than that I can see where you as Private Practice 
would need to keep records of your clients, just like a private doctor's office.  
The and/or to mother or healthcare about baby would be just that... to the 
mother since it is an and/or statement.  At least thats how I interpret it.  Glad 
to hear to feel it is an improvement.  More autonomy, less accountability to 
the health care maze/craze.
