Can anyone here explain exactly what action (baby or pump) that might cause
nipple bruising (right breast) at the 10, 11, 12 o’clock position? Not a
stripe or line, but rather what looks like darker pigment ‘blotches’?

 

Twin boys born at 37.1 weeks via induction = epidural = c/sec.  5.12 and 4.1
pounds at birth.  Medical concerns at birth (mother unsure of what,
communication with hcp’s was not optimum, but did include both babies having
tremors, which was explained as withdrawal from her antidepressant meds
[prozac and wellbutrin]), both spent at least three days in incubators,
formula/bottle fed from start.

 

Babies discharged on day 6.  I first saw babies at home on day 8.
Characteristics (glassy eyes, no fat pads on cheeks, low muscle tone,
inability to stay latched) were still very premature.  Mother is pumping
with hospital loaner pump, most mostly supplementing with formula, between
50 – 100 mls total per day over two or three feeds.  I helped refine latch
and suggested other positions, which seemed to help.

 

Now, 12 days post partum, both have regained birth weight and are
breastfeeding, with supplemental bottles.  Mother intends to wean off
supplements as soon as pedi give the okay (hopefully at tomorrow’s
appointment).  Babies still look and act very premie, even the bigger babe
who is nearly six pounds.  Mother says nipples are a ‘little’ sore (about a
2 or 3 out of 10).

 

Other than the bruising, the nipples appear normal and healthy.

 

So, do you think the bruising will just resolve itself once the pumping and
bottles are withdrawn?  Mother is not asking for help, nor complaining of
pain that she feels needs to be addressed.  She is just ‘curious’.
