Okay, this one is really "out in left field" (baseball analogy for being 
far out of the norm). I had one toddler in my practice refuse to bf and 
refuse expressed milk (even made into ice pops) during chemotherapy for 
leukemia. Once the doctor realized that she was probably nauseous when 
she laid in mom's lap to bf, and probably associated the taste of mom's 
milk with the nausea, he gave baby an anti-emetic and she went happily 
back to bf and taking her milk ice (for her sore throat).

That's an N of 1 in 25 years as an IBCLC and almost 30 as an LLLL.
