Lisa,
"Well flanged lips" are a sign of a shallow latch.  A properly attached
newborn will be so close to the breast that one cannot see the lips at
all.  Look up info on asymmetrical latch, and try that with this dyad.
There are some good articles from Leaven on LLL's website, including one
by Andrea Eastman that quotes Diane Wiessinger extensively, and has
great photos.

If mom's breasts are full, and her skin was already stretched by
implants, and the breast is abnormally firm from implants taking up
space, it might be more difficult for baby to grasp as well.  Having mom
pump regularly, and feed very very often can help keep the pressure from
reducing mom's supply.  I suspect that implants increase the pressure
effects of engorgement, and moms in my practice with implants (all of
whom bf successfully) needed to bf often, more often than moms without
implants would.  When returning to employment, they also needed to pump
more often than the standard advice too.
