I had a mother of twins exclusively pumping for them.
The most impatient one developed green stools when the mother, not wanting
to keep him waiting, gave him the milk just pumped, without waiting to
finish emptying the breast, for several feeds on a row.
After she started waiting and mixing "first" and "second" milk, the
symptoms disappeared.

Could it be that the mother has been pumping first and breastfeeding after
for a few times, for any reason?
Just one idea.
