Have one for the collective wisdom:
Three week old baby.  He and mom have already overcome:
1.  being born at the most baby-unfriendly hospital in town
2.  emergency C/S
3.  delayed early feeding b/c of #'s 1 & 2
4.  tongue-tie (I clipped on day 9, did nicely)
5.  sleepy early, b/c of #2
6.  mom's very flat nipples
7.  jaundice that required bili-blanket, b/c of #'s 3, 4, 5, & 6
8.  mom's compromised milk supply, b/c of #'s 1, 3, 4, 5, 6, & 7  (milk
supply now back to meeting baby's total needs!)

Baby can now latch pretty well, but mom still with sore nipples b/c he
clamps.  Have been feeding EBM with Nuk.  Have been using pacifier (against
my advice) b/c mom is a pedi dental hygienist. "You can take a paci away,
but you can't take their thumb away."

He's not nipple-confused, as in, he makes appropriate suckling motions with
tongue, doesn't thrust, etc.  But, I think he's "source of milk"-confused,
and thinks he has to clamp to keep it from drowning him and/or to keep it
in his mouth.  Mom with large, pendulous breasts, but gives good support
when nursing.

I suggested using a tube at breast to give all supplements, and getting rid
of the pacifier. I think since he can control flow better from tube than
from Nuk nipple, maybe he won't clamp.  What else can I suggest to this mom?
