Can anyone direct me to clinical patient references or treatment options for a 34 y.o. pregnant client with familial chylomicronemia, an auto-recessive inherited very rare inborn error of metabolism in which no lipoprotein lipase exists to clear fats from the blood. Consequently, even on a very low fat diet, triglycerides build up to extreme levels leading to pancreatitis. 

Client was diagnosed at age 10 after 3 pouts of pancreatitis. She has been on an < 15-30 gram/day fat diet, and avoided all alcohol which has controlled her condition well. She has had about 4 other pancreatitis episodes to date when off this very stringent diet. 

Her first challenge will be a healthy pregnancy due to a steep rise in estrogen which can raise triyglycerides even in "normal"
pregnant women.

Her second will be breastfeeding.  I know she badly wants to breastfeed as her mom is a Lactation Consultant/former La Leche League Leader. She was herself a longterm breastfeeding toddler.  I am her mother.

The only info I have been able to get is that her breastmilk will have an absence of long chain fatty acids.  

Any information will be greatly appreciated.
