Dear Collegues,
I have a client, I could use your expertise. Mid Eastern dissent, delivered g5 p4, repeat C/S birth. Baby was transferred to another facility. I set mom up with a hospital grade breast pump and pumping until discharged. Baby discharged same day as mom. Delivered 14 days ago. She had band
surgery for weight loss prior to pregnancy. Lost 50 lbs. Band deflated during the pregnancy. States not eating or drinking few days after going home. States no appetite. Encouraged her to drink sips of fluids and small meals such as she did when she first had her surgery so that she could get enough nutrition. Hemoglobin was 9.8 received a script for iron but never started taking it. Offers baby the breast but states not even a drop of milk. She has to supplement 2 ounces of formula. She refuses to use a breast pump states it is too uncomfortable. Did not breast feed last two siblings didn't want to but wants to breast feed this baby since he is her last. She wanted to start taking Reglan since Domperidone is not available in this country or should I say not affordable to her from compound pharmacy. I encouraged her to try to hand express between feeds as well as nursing baby and increase her fluid and dietary intake. Increase of protein and whole grains.
Do you think it is possible for her to bring in her milk supply. She was also on antibiotics for 14 days for wound infection. Baby on antibiotics for resp. aspiration.
