Re the 16 yo with copious bloody secretions:
I think it is more noteworthy that the mother is leaking so much
spontaneously within the first day or two. Not necessarily abnormal, but
definitely unusual in my experience. The bloody part of it, while it can be
disconcerting to mom and staff alike, is fairly common, and I think is a
result of a large volume of milk and congestion in the breasts. Did this
mom, by any chance, experience a large increase in breast size during
pregnancy? Perhaps she is going to have a very abundant supply. She may have
some sort of prolactinoma in the pituitary, or even bilateral intraductal
papilloma(s).
But it could just be rusty pipe syndrome,  to a greater extent than usual,
due to the large amount of colostrum present. I would encourage her to
breastfeed as per the usual good breastfeeding management practices, and
keep an observant eye on her and inform her primary health care provider to
followup as well. Please report back to us, if possible, on the outcome. In
cases of rusty pipe that I have seen, they generally have resolved in a day
or two.
