Our group (Elad, Botzer, me and others) has used ultrasound to track
tongue movements in an objective way, specifically in babies with more
subtle ("posterior") tongue tie before and after frenotomy. We do see
specific differences that impact feeding. We are working on applying the
analysis to some other issues, and are working on publishing. It is a
very mathematically intensive method, involving tracking the tongue at
28 points from front to back on every frame of a few suck cycles on
ultrasound video. Read Elad's paper on normal suck for a better
understanding of the method: Biomechanics of Milk Extraction During
Breastfeeding, Proceedings of the National Academy of Sciences (PNAS),
April 2014.

What you see is also determined by where the transducer is held. And how
still the baby stays, and if the tech sneezes, etc. Science is difficult
to do well, and no one is prescient, there are always surprises that
cause you to need to go back again to look everything over again.

In the meanwhile, I strongly agree that we need to work on good bf
management first, and then consider other interventions.
