Long ago, many years ago on lactnet, I think we had a  discussion on this.
Some consultants size for the baby's mouth, some size  for the nipple. It
makes more sense to me to size for the nipple. I know  many mothers try
various sizes, and sometimes LCs try various  sizes.>>

 
I think you need to size to the mom's nipple.  What good does it  do if the 
shield is too small, the milk ducts in the nipple and just beyond the  
nipple are compressed and no milk can be released?  I know of one mom who  
bought a shield at Target, getting the smallest size because her baby was a  
newborn.  Came into the clinic not gaining -- baby transferred about 6 ml  with 
that shield.  Put a larger shield on, and the baby was able to  transfer 
about 1.5 ounces with the larger shield.  Fortunately mom had been  doing some 
insurance pumping so her milk supply was good.
