How long has this mom been doing one-sided feedings this time around?  I would certainly give that at least a week to kick in before I thought about suggesting more intervention.  It sounds like she just should have stayed with nursing on one breast per feeding.

If that doesn't work, I would definite go with sage before resorting to something as strong as estrogen.  She should start slowly and then build up the dose if there is no response.  Sage can be eaten - heavily seasoned stuffing or rolled up in a piece of a pb sandwich - or drunk as a tea or she could use a tincture.  Estrogen containing contraceptives are really a last resort for moms who have exhausted all other alternatives.  I would probably try pseudaphedrine before estrogen, but that, too, can lead to a very sudden drop in supply that reportedly, is sometimes not recoverable.  So, be careful.
