An idea I often share with Moms who are applying heat following breastfeeding due to Raynauds is to keep a heating pad next to the nursing area with a cloth on top.  When the baby comes off, they can grab the cloth and put it right on.  I also prefer applying dry heat  because it doesn't cool as much as a moist cloth.

Recently our office received samples of a breast pad filled with a gel and a disk that could be clicked causing the gel to heat up almost instantly.  (it can be reset later) It is a cool idea, (or maybe I should say "hot" idea) and sounds promising for something like this, however one Mom who tried the sample told us the gel became too hard and less comfortable - so perhaps it needs to be refined first.
