Here we go again with another question on meds... Infant is a 7 mo. old
nursling. We had to work a bit to get him started as a newborn and he has
done well since week 2 and loves to nurse. He's on a small amount of solids
and has been takin 1-2 bottles of ABM for the last 6 weeks. Mom was recently
diagnosed with a chronic condition which requires steroids. Currently she is
taking 8 mg. Prednisone/day. Dr. wants to switch her over to Indocin (25 mg.
3x/day) to see if she can use this instead of Prednisone. The steroids and
her condition have caused her to be depressed so Dr. has suggested she take
Lovox at nite. Mother checked with a highly reputable drug consultation line
Monday about the Indocin and the consultant said Indocin was contraindicated.
We called back the drug consult line together after I checked Dr. Hale's
book and the AAP list and the consultant put us on hold for a couple of
minutes and then backpeddled her information saying she had read from
Micromedics first and agreed that AAP should probably be the final say.
(Needless to say, I was very disappointed that we got conflicting
information from the same source). Of course, this mother was charged for
both phone calls. My question is: can she take Prednisone, Indocin and Lovox
at the same time. How do other LCs deal with these complicated questions. I
would assume that even though two drugs may be safe in isolation for a
nursling, could there be a danger in an interaction of two? Many times the
doctors are not interested in researching the information even though the
mother really wants to nurse but certainly doesn't want to put her infant in
jepordy? Dr. Hale, this is my third drug question...I feel like I am
wearing out my welcome but after this experience with the drug consult line,
I am not sure I can trust anyone else...
