My suggestion in this situation is that too many things were tried within a
short space of time without allowing the mom and baby time to establish
breastfeeding well. "Only" pumping 8 times a day and feeding the infant 12
times a day and giving formula is alot of work--a true killer of desire to
breastfeed. If infants are expected to lose up to 10% of their weight and we
usually allow them 2-3 weeks to regain that weight, I wonder about all these
interventions. I also consider 8ozs. of formula alot of formula in this
situation. If we are going to give formula anyway and the mother is refusing
an SNS, why make the mother kill herself by pumping? (oh-oh, I'm sure that
statement won't please some people). Mothers need to be spending time with
their infants to breastfeed. If they are giving formula and pumping, they
are spending enormous amounts of time fiddling with equipment and not doing
the thing they need to do. Which is spending time with a baby at the breast.
Introducing equipment comes at enormous cost to mothers and infants in terms
of interferring with breastfeeding. Mothers end up establishing a
relationship with their equipment. I realize every case will be different
but I think sometimes we have to have faith that breastfeeding works. Yes,
maybe she is the 1-2% of women who cannot fully breastfeed her infant. But
chances are she isn't and I think we need to approach all mothers with that
in mind. Valerie W. McClain, IBCLC
