Is it one side or both that has pain? It could be vasospasm, it could be
tongue tie, I know you've thought of those, it could be a small crack or
fissure which is very painful, particularly if the base of nipple is pulling
from the areola. I would closely check positioning and latch, sometimes baby
is pulling or twisting on the nipple due to poor positioning, and often
experienced moms are the ones who don't pay too close attention to this, but
just "wing it."
