I am guessing (and hoping) that the mothers were assigned to groups based on
their stated feeding preference: exclusive vs mixed breastfeeding in
hospital. I assume this because it says mothers volunteered to participate.
I don't think they were randomly assigned, because I don't think that would
have passed institutional review. Perhaps this is clarified in the full
article.
