In South Africa we *rarely* have access to "hospital grade" pumps.  Many
mothers use a manual pump for many months with great success.  Lactinas have
become available fairly recently.  Our maternity unit has ONE.  It sat in a
cupboard for a while!  Most midwives are scared of it because they don't
know how to put it together.  Recently we had three mothers providing EBM
for their prem babies.  One used the Lactina (single pumping), another used
a manual pump and the third produced buckets with hand expression!  The
biggest problem is mothers not pumping frequently enough in the beginning.
