Ii have followed premies diagnosed with renal reflux who have been on
prophylactic antibiotics for extended periods of time (many months).  In
fact, one ended up with GI candida that caused three hospital admissions
before it was properly diagnosed.  I posted on Lactnet about that situation
about a year ago.  I would encourage any parent going along this route to be
certain to protect the flora of the gut.

I think that the assumption is that any newborn with kidney abnormalities is
going to get an infection.  An exclusively breastfed infant would likely
have less chance of infections in general, but I don't know about renal
disease in particular.
