There are cases where Baby B is in danger from Mother A's milk where
Baby A would thrive with it.

If Mother A is CMV positive and has been throughout the pregnancy, her
baby should have no problem with her milk. BUT, if Mother B is CMV
negative and Baby B recieves Mother A's milk Baby B may have extreme
problems because of this accident. He has no immunity from his mother
and he is already a sick kid.

CMV is a common viral infection. This is a real danger as much as we
would all love to think no harm can ever come from mother's milk.
