I am thinking back to my recent sample of one (my granddaughter and great
grandson), and I agree that continuing the use of a nipple shield after
frenotomy until all pain, internal bruising, and signs of nipple
compression are resolved would definitely be something to consider. 

Many mothers do experience immediate relief of latch difficulties right
after frenotomy, especially if done in the first few days. However, if
there is a delay in the clipping, and the nipple has been traumatized
and/or milk tranfer needed assist with a shield, and possibly the baby
has learned clamping and other bad habits trying to compensate for the
limitation of tongue motion, it may take a few days or so after the
tongue is clipped for mom's nipple tissue to heal completely and feel
comfortable, and possibly even more for the baby to "unlearn" those
habits, and perhaps benefitting from some "tongue games" to retrain a
bit. It may be a mistake to lead the mother to believe relief will be
immediate, and if her motivation has been worn thin by the experience, it
may be more realistic to err on the side of encouraging her to use the
shield a little longer rather than being too hasty to discard it.
