I was involved in a discussion about this recently -- maybe on the hospital
based LC online group I'm on.  I think that the majority of LCs do not use a
consent.  Or if they do it is in the form of an instruction sheet, including
follow up and returns for baby weights if indicated.
When used in appropriate situations. nipple shields can keep breastfeeding
going!  We don't use a consent, but do try to continue to follow up with
these moms and babies -- phone calls, return for baby weights, etc, to make
sure that baby is thriving and gaining weight -- possibly
instruction/assistance in getting rid of the shield, but sometimes moms just
keep using them.  As long as baby is growing and thriving, I don't see it as
a problem (other than the nuisance of mom having to wash it and find it when
she's ready to breastfeed).
