The local news station in Chicago (WBBM/CBS) has been carrying a story about a premie who died of meningitis linked to powdered cow's milk based formula (no brand name give).  This occurred in the hospital during the baby's stay in NICU after birth.  The baby was one of a set of twins and the other twin survived.  
 
I remember the discussion on Lactnet a few years ago and have always warned parents about powdered formula for their very young babies when their pediatrician has ordered them to supplement breastfeeding.  Wasn't this generally accepted information?   I thought that there was a FDA warning associated with use of powdered formulas because they carry bacteria.  I was sad to hear that these products are still in use in hospitals and came to this sad result.
