Diane,

It is entirely possible that this mom's problem could be related to
perimenopause.  The sympotmology she describes is right on... hot
flashes, PMS sympotmology, reduced milk supply.  Some of us just start
the change sooner than ohters.  Being diabetic doesn't help matters,
though, as insulin levels play a big part in hormone production and
regulation.

It may be wise to try to encourage her to start waking the baby for at
least one nursing at night, if she's willing.  This may help to regulate
her hormones a little bit, plus help her to produce more milk, as you
know!  Her fatigue may prohibit her from doing this, but suggesting it
can't hurt.
