At our hospital we (the LC's) make follow up appointments for all dyads that 
we feel will need f/u assistance.  This appointment is made prior to their 
discharge (d/c) and is written right on their d/c planning sheet.  3/4 of 
our out patient consults are made through these appointments.  We also have 
our on going 1x/week support group for all moms and baby's.
