Betsy, I have seen this more with moms who really don't want any help
because they have already breastfed 3, 4, 5 other children, and just come to
me to get the name of a cream for sore nipples.  One mom was breastfeeding
her 11th child, and had forgotten the name of the cream.  She claimed that
with each baby she climbed the walls with pain for at least 2 months, but
after that went on to breastfeed them for at least 2 years.  I did not want
to tell her the name of the cream without seeing the way she breastfed, but
she insisted that she is an experienced mom and just needs cream.  I
pretended not to know, and promised to come and tell her.  I waited until
the baby was in with her, and lo and behold, the problem was horrendous
positioning: the baby was so low in her lap that he was almost on the floor,
and he was pulling the nipple down, and causing the skin which connects the
nipple to the areola to gradually separate.  I grabbed a pillow, lifted up
the baby and put the pillow under him to raise him to breast height, and the
mom almost fainted with disbelief that there was NO PAIN!!  She walked
around in shock all day that there was no more pain.  I wish all cases were
this easy.
I call this the multipara position: she sits slightly forward, her knees are
open, and the baby is hanging down there between her knees pulling the
breast down and the nipple with it.  With each baby the position gets worse
and worse.  (I have to photo it.) The result is a nipple that looks as if
someone has taken a razor and is trying to cut it off the breast.
