IMO, blood loss may have something to do with low supply. {Retained placenta
can also affect milk production. }
Did mom have any problem getting pregnant?
Is this her first baby? Any problems with other babies breastfeeding?
What else is going on in her life? Stresses?
Has mom had her thyroid checked?
Is she using the Birth control pill?
Your suggested protocol sounds great,  but she may never produce enough
milk.
Why has she been sterilizing after every use of the hand pump?
What about herbal remedies such as fenugreek and blessed thistle? (2
tabs/3xdaily)
Four weeks is a good length of time to work on establishing a milk supply,
but it wouldn't hurt to take a few tests and try a few more things before
"giving up".
