Our babies in the United States all seem to have some form of GER!  But a
diagnosis is made with an upper GI.  For instance my second nephew (my brother
and his wife's second child) came home from the hospital and sounded like he was
drowning after his feedings.  He then started projectile vomiting.  They went
to the Ped. the next day and he had an upper GI and sure enough he had a
pretty bad case of Reflux.  He now nurses on one side at each feeding and he is
growing great and spitting vs. vomiting.  He did have to go on Reglan and Zantac
for awhile because he was such a miserable little guy.  But that has
improved.

When a mom comes to me at support group or for a consult you can usually pick
up on these babies who haven't been diagnosed.  They arch and cry out at the
breast.  Sometimes it is just with a MER but other times they are feeding
peacefully and then it starts without any relationship to a let down.  But most
moms do not even want to get it diagnosed, they manage their breastfeeding and
use comfort measures and the babies outgrow it.

NICU babies are famous with GERD.  We have one now that literally tries to
die with the spells from time to time.  He has been "bagged" several times for
several minutes until he starts breathing on his own.  The frustrating part???
How often does the NEO have him feeding, Q 4 HOURS!!  I just don't get this
ignorance of how to feed these babies.  And the poor Mom is terrified to try
nursing again, one of the spells happened while he was at the breast.  So
tomorrow we will see if we can try again.  Thank goodness her supply is great.

Where today I worked alone, saw one consult, did rounds in NICU that lasted
an hour, answered 18 phone calls, saw four NICU moms (wanted to see all 23 of
them...but not enough hours), one I spent over an hour with, due to her almost
dying and after 6 days is finally able to start nursing/pumping AND no habla
ingles, I saw ONE mom out on the regular Post Partum floors out of 40 moms!  I
worked 8:30-4:30 and I only took a 15 min lunch....at 2:30 PM.
