i had a client recently who prepared for the arrival of an adopted baby by
pumping infrequently, taking my tincture, and taking domperidone. on the day
of the baby's birth, she got 2 ounces TOTAL from several pumpings that day.
she pumped even less (and not at night at any point) during the next three
days. on the evening of day four the baby came to her. he had been breastfed
by the birth mother and not had any other nipples.

the adoptive mother immediately put him to the breast, he nursed beautifully,
and she then did not take him off. slept with him, nursed him constantly. i
weighed him every day, examined diapers, etc., and he did not lose any weight
during the next four days, had good output, and on the fifth day he had
gained almost an ounce. during this time she had given him one ounce on day
seven (his age from birth), and two ounces on day eight, of her previoiusly
pumped milk (by tube feeding device). so, she was able to produce all the
milk her new baby needed by doing what i suspect is done in new guinea and
not routinely done here, constant baby stimulation.

i was really looking forward to seeing how it would go from there, but the
baby was repossessed by the birth mother the next day.
