When you give the mother the amount of time to pump did you mention that the timing starts when the first drop of milk comes not from the first pumping.  Sometimes it takes more several minutes before the milk starts flowing.

Concerning double pumping, I have always been told that a mother gets a better letdown when she pumps both sides at once or else nurses one side while pumping.
