RE/ the baby pulling the nipple through the shield, Does this 
mother have a long side or shank on her nipple? This can cause 
the nipple to be pulled in too far and also make it difficult for 
the infant to latch deeply enough to access milk. I had one 
patient with a nipple so long that the baby needed to grow  
before being able to nurse effectively. The supply needs to be 
protected until that time. Just a thought you probably assessed 
this already.Chris Erland
