Nikki Lee posted on the Cochrane review of research on breast stimulation to
start labor.  These studies were done to determine whether using nipple
stimulation is an effective and safe way to induce labor when the pregnancy
has continued beyond the point at which the woman would be induced.  They
were not done to determine whether nipple stimulation could start pre-term
labor.  I've only been skimming LN the last several days and don't recall
who noted that the uterine muscle's oxytocin receptors seem to be
insensitive to oxytocin until labor is meant to start.  Thus, a woman who
has not been advised to avoid any situations involving peaks of oxytocin,
such as orgasm, could be assumed to be a candidate for hand expression of
colostrum.
Many women experience spontaneous leaking of colostrum in advanced labor.
If it is anticipated that the baby will need extra help to obtain enough
colostrum, it could certainly be safely collected during labor, if not
before.
BTW, I would be **very** hesitant to suggest that anyone with symptomatic
placenta previa do extra nipple stimulation.
