Dear Carole,

I learned to use the SNS in a way that avoids the formation of a vacuum.
Since there are two tubes, one is the "feed" tube taped to the breast and
the second is a "vent" tube taped end-up against the bottle. As the  baby
nurses, you may open the feed tube first, then shortly thereafter, open the
vent. At the end of feed, close the vent first, and then the feed. That
extra precaution prevents milk (especially if it is on the cool side) from
jumping up the vent and slowing down flow again.

That way, if you want to provide less flow, you have the option of one of
several choices. Either go down in tube size, lower the bottle in relation
to the breast, or you can close the vent (like gently applying the brake to
slow down the flow.) If you do pinch off the vent, you will need to open it
briefly at baby's pauses to keep a vacuum from happening again. The latter
is for the more advanced mother only, or for the LC to assess the
appropriate tube size.

One thing  to note is that Mom needs to assign one tube as the feed tube,
the other as the vent and keep it that way throughout the feed, whether she
uses one or both breasts.

Use of the SNS can be quite tricky.  But when it works (especially with the
motivated Mom), it  works well.
