Have seen Paget's. It looked like a cratered nipple at first, was very 
painful, red, open circular wound in the center of the nipple, as 
opposed to thrush which usually is more severe around the circumference 
of the nipple.

In later stages, the Paget's looked white and crusty across the entire 
face of the nipple.

Culture will find any yeast or bacteria, and a punch biopsy is indicated 
if normal treatment does not resolve the wound.
