I like flipping the sides back so I pull up a little bit more breast/nipple into the shield.  It keeps the shield from sliding around.  Sometimes I have the mother soak it in hot tap water for a minute or two, since it seems to help it stay on more securely.  I think it does help especially when the nipple is shorter.  I do work mostly with premies in the hospital, so that may be why this works for me.

This topic intrigues me.  I have heard of a few ways of putting on a nipple 
shield, but I have not seen that it makes any difference, in practice, in how 
the nipple shield is placed.  Maybe it depends on the mom's nipple size to begin 
with.  I've had moms turn them halfway inside out and placing over the nipple, 
turning right side out and tugging on top and bottom and the shield just falls 
off and the mom picks it up and plops it right back on and it looks the same.  
I've also had them turn them completely inside out, but without a nipple, or 
with just a smallish nipple, the end results looks the same.
   
  I imagine that if the mom has somewhat a nipple to begin with and the smallest 
nipple shield is used, the nipple theoretically could be pulled slightly into 
the shield because of the drag from the sides, but there cannot be any vacuum 
since there is holes in the end of the shield.  What is the theory behind these 
"technogical" ways of fitting, because I'm not seeing any difference in where 
the nipples ends up or how the shield sits?
