Moms who are resident physicians have similar concerns.  They are
breathing in toxic anesthetics and exposed to germs, and working 36 hour
shifts.  A good electric breastpump is essential.  I advise moms to pump
twice in the morning, then as often as they can throughout the day.  Mom
can do some extra pumping in addition to feeding on her days off to try
to compensate for any shortfall in milk during the day on duty.  Dr.
Judith Schreiber (chief scientist in the NYS attorney Generals'
environmental toxicology division) says that bf moms should have their
duties modified to try to avoid exposures, or else they should at least
shower and change clothes before coming home.
