In ten years of bf counseling I had one mom who just never produced milk.  She
was referred to me by an LC in town.  Baby was just 6 days old when I saw him,
full term, vaginal birth.  No excessive bleeding.  I pulled my hair out over
this one.  OB checked her for retained placenta fragments and found none.  No
history of infertility.  Thyroid levels were fine.
The mom pumped, took Reglan and fenugreek.  The baby was being fed with an SNS
at the breast.  Her mother (the grandma) had bf her own children and had been
a LLL member in her day, so she was delighted I was there helping her daughter
and grandson.  Very supportive atmosphere.  The poor mom was flabbergasted
that she couldn't produce milk.  She had just always assumed she would bf her
babies.
The one thing that I did note was that the mom's breasts were an odd shape -
long and tubular.  I remembered something from a conference and dug through my
notes.  It said that tubular shaped breasts could be a sign of insufficient
glandular tissue in the breasts.
The mom tried for over a month and never produced milk and finally gave up.  I
reminded her she could continue to feed the baby at the breast with the SNS,
but she was tired of the whole thing and just bottle fed.  I can only assume
that this was a true case of glandular insufficiency.  I hope you find a
solution for your mom.
