Hi Lactneters,
I hope someone out there can help us with this case.  I am not sure Pat's
message got through to you, so i'm forwarding it.  Since Pat spoke with
this mother on 10/5, the baby continues to vomit, as usual, but is happy
and thriving.  He also continues to have the green, slimy stools.  They saw
the GI man yesterday and the only abnormality they found was  6 acidophilic
cells.  Mom asked if that was alot and the  GI man said that usually there
are more than 20.  He suggested she put the baby on alimentum and Reglan.
He felt that the problem was reflux and that the bloody vomitus  was a
result of irritation.   Mom does not want to stop breastfeeding if she can
help it and was afraid to start him on Reglan after having looked it up on
the internet and seeing all the possible side effects.  She is going to
wait until she sees her pedicatrician on Tuesday and get his/her opinion.

Sure would appreciate it if any of you have had experice with anything like
this.

I am a lactation consultant who received a call today with the following
details.  Please evaluate and advise.

6mos old male, delivered via cs at term.  This is an exclusively breast fed
infant who has vomited since day 3 (not projectile, but also not just spit
up...enough each time to reach the floor) .  Vomiting occurs after and
between every feed and, perhaps surprisingly, the infant has thrived.  His
weight at birth was 9-13 and at 6 mos he is 19.8.  Developmentally he is
also thriving, sitting, rolling over in both directions and babbling
constantly.  Last week, an UGI ruled out pyloric stenosis.  An endoscopy
revealed no ulcers, or sick gut.  Biopsies of the esophagus, stomach and
lymphnodes are pending.  Up until last week, stools were normal.  At that
time they became greenish and very slimy and mucusy.  Mom eliminated
lactose from her diet 1 week prior to that change in stool and continues to
be lactose free.  She has also elimiated caffeine and nuts.  The infant was
introduced to rice cereal but after 3 days, when mom learned that the tests
would all need to be done, she ceased offering it.  On Monday, at the
specialist for the endoscopy, the infant initially vomited pink tinged
vomitus.  At the end of the visit, the infant vomited frank blood.  Mom
says that sometimes the vomitus appears mucous encapsulated blood and now
there are green chunks.

Also, at this time the infant was weighed (on a different scale than
previously) and appeared to have lost a pound.

My understanding is that galactosemia usually appears early on...?  Is
there another possible metabolic disorder that might be occuring?
