What does the nipple look like, ie. abraded etc?
Is she using the "off center" latch?
Is one position less painful than another?
What is the swallowing pattern?
Does pain and/or swallow freq improve with compression?
Thus, are you sure about latch adequacy as pain coupled with low volume
suggest otherwise!!
While concentrating on all of this, would try a few days of ibuprofen as well.
