Subject: Re: [LACTNET] 6 mo old with 0 weight gain last 2 mos

Dr. Jack Newman recently sent out the following email titled "late onset
slow weight gain" to be distributed widely, I post it below since it may
apply to this situation. If you want the handouts he mentions, email me and
I'll send them, since we can't attach handouts to this list.

Dear all,

More and more I am hearing from and seeing mothers at the clinic with their
babies who were gaining very well for a few months and then slow down.
Please see attached handout on slow weight gain after the first few months
for reasons this might happen. See also Protocol to increase breastmilk
intake by the baby.

Unfortunately, too often, the suggestion of the physician is to give
formula. This is unfortunate, because most mothers will give the baby
formula by bottle, thinking that since the baby is older, this will not
result in the baby's refusing the breast. This is absolutely untrue. In
fact, at the age of 4 months, say, the baby is more likely to refuse the
breast.

Babies will figure it out very quickly. If they are hungry and they get
slow flow from the breast, and then get a bottle, they will figure it out
very quickly. I have seen several mothers whose babies are already refusing
the breast after only 2 or 3 days of supplementing, even if the babies never
got bottles in their lives before. It's not the baby who is confused.

In any case, why give formula when at 4 or 5 months the baby could be
started on solids? Formula could be added to the solids if it were thought
absolutely necessary.

Solids will increase the calorie intake of the baby. But how to increase
the breastmilk? See Protocol to increase the breastmilk intake by the baby,
but if that doesn't work quickly, domperidone can be very useful. See
attached handouts.

See in Protocol to increase breastmilk intake by the baby how to know the
baby is actually drinking milk (not just sucking).

Please do what you can to help the mothers and babies continue
breastfeeding.

Jack Newman, MD, FRCPC
