Hello,

My name is Rebecca Plumtree and I have a Certificate in Breastfeeding.
Currently, I am working on my hrs at a local breastfeeding clinic with
an IBCLC.

Our question is this:  Are prolactin receptor sites permanently limited
when there are retained placental fragments up to and possibly beyond 4
weeks pp?  Can anyone offer an explanation?

  We have a mother who  expelled large clot about 2.5 weeks post
delivery and still has not seen an increase in milk supply.  An
ultrasound shows fluid in the uterus.  Mother and MD both do not want to
discuss retained placental fragment possibility.  Mother is only able to
express (via pump) about 3-4 ounces and is supplementing with formula,
all at breast with SNS.  Also taking usual galactogogues.  Baby is now 4
weeks old and skinny, though gaining now that formula is being used.

At this  point, will the milk supply ever come up?  Any knowledge and/or
suggestions most gratefully received, ASAP.
