Judy,
Are you confident this pair have thrush, b/c you do not mention  
symptoms of such, but mom and babe are being treated accordingly.  
Have you evaluated this babe for tongue-tie? If this is definately  
not the cause, then my first step (other than those you have taken to  
protect/increase supply and address pain) would be a cranial-sacral  
therapist. If it is a tongue-tie, my second step, after the clipping,  
would be the CST.
