Everything about this case suggests massive oversupply issues. Drowning the
baby and herself during feeds, green stools, vomiting are the sx I see with
oversupply. This baby cannot tolerate the lactose load he is getting. I would
do a test. Pump until the milk looks creamy and then put him on the breast.
Do this for a day or two and see if he gets better. Make sure he is getting
hind milk.  If he passes this test, work diligently on bringing the supply
down.  In these cases i like to do one thing at a time.  If that doesn't
work, work on dietary measures, but no matter what else is going on it seems
pretty clear that this mother has an overabundant milk supply.

I agree with Kathy.  Also if the time between feeds on  
one breast get too long mom will be set up for mastitis.   
Erring on the side of caution and getting the hindmilk in the  
baby and the supply down is ideal.  

P.S.  How do I get the msn sign from coming up every time I post.
