I don't think this is something to be too casual about.
Inflammatory breast CA comes to mind.  I understand it starts with
red areas that aren't necessarily tender in early stages.  It's the
worst kind in terms of outcome statistics so should be identified as
early as possible for best chance of treatability.  The nipple
retraction is suspicious for something happening within the breast.
A needle biopsy at this stage might not be such a bad idea.  If it
turns out to be nothing, there's plenty of time for healing before
the baby is born.
Hopefully it is nothing, but the alternative is serious enough to be
ruled out.
